• To incorporate your organization, please go to http://business.illinois.gov/registration.cfm  or go to your nearest Secretary of State’s office.

Insurance

• Insurance Coverage Outline
• Insurance Certificate Request Form
• Insurance Claim Forms – complete and submit for injury along with the online Pullen Insurance Form at https://two.pullenins.com/claim/overview.asp?INSID=IL
• Incident Report – to be completed by league administrator to document an incident/injury in case of a lawsuit
• IYSA Medical Release & Waiver – required for every participant Coach keeps original in case of an injury
  - League keeps a copy on file to submit with insurance claim
  - League retains a copy until the participant has reached 21 years of age
  - Since the waivers contain private information, store them in a safe locked box
