Incorporation Information
- To incorporate your organization, please go to http://business.illinois.gov/registration.cfm or go to your nearest Secretary of State’s office.
Insurance
- Insurance Certificate Request Form
- Insurance Claim Forms – complete and submit for injury along with the online Players Health Claim Form at https://app.playershealthclaims.com/landing/illinois-youth-soccer-association
- Incident Report – to be completed by league administrator to document an incident/injury in case of a lawsuit
- IYSA Medical Release & Waiver – required for every participant Coach keeps original in case of an injury
- League keeps a copy on file to submit with insurance claim
- League retains a copy until the participant has reached 21 years of age
- Since the waivers contain private information, store them in a safe locked box
