14U Play-In Game Change Form - Illinois Youth Soccer

This form must be submitted no later than 5pm on Sunday, October 6.

Teams can agree with their opponent to move their games to an alternative date and alternative location other than October 19-20 in Rockford.

BOTH teams must be in agreement. If teams do not agree the match is played in Rockford as scheduled by IYSA on October 19-20
If the outcome of your match impacts another play-in game you must have an agreement with ALL teams involved in moving your match to later than October 19
Moved games CAN NOT be played on October 19-20 at another location. If your teams want to play October 19-20 you must play in Rockford
All agreed upon changed games must be played by November 10
IYSA will send all referees for moved games
